At The Potential In Us, we believe that housing is more than just a place to live- it's the foundation for independence, health, and community connection. That's why we're proud to provide housing support services to eligible individuals under New Jersey's new Medicaid program.
What is the housing supports program?
This program provides services to individuals who are at risk of homelessness or experiencing homelessness, transitioning from institutionalization, or housing instability. This Medicaid-funded programs aims to promote community-based living and reduce New Jersey's homelessness epidemic.
Who Qualifies?
To be eligible, individuals must meet the following general criteria:
- Be Medicaid eligible and enrolled in a Medicaid managed care organization (MCO)
- Be at risk of homelessness, housing instability, or transitioning from a state institution
- Demonstrate a need for housing-related support services to achieve or maintain stable housing

At The Potential In Us, our experienced team provides individualized housing support tailored to each participant’s goals, preferences, and needs. From finding the right home to developing the skills needed to maintain housing, we’re with you every step of the way.

Pre-Tenancy Services
We support individuals in planning and preparing for housing, including:
- Housing goal setting and person-centered planning
- Navigating the housing application
- Assisting with housing searches and viewings
- Helping complete rental applications and gather documentation
- Communicating with landlords and property managers
- Seeking ongoing rental resources and support
- Coordinating lease signing and move-in logistics
Note: The Potential In Us is able to provide financial support for a number of pre-tenancy and tenancy-sustaining needs.
Tenancy-Sustaining Services
Our support doesn't end after move-in. We provide ongoing services to help individuals remain stably housed:
- Landlord and tenant relationship support
- Setting up utilities, rent payment systems, and service connections
- Helping secure basic furnishing and household items
- Budgeting and money management coaching
- Addressing lease compliance and maintenance issues
- Life skills development (i.e cleaning, grocery shopping, transportation)
- Assisting with housing recertification process including lease renewals and housing subsidies renewals
- Connection to social services
